4. Aadhaar Card (FRONT + BACK) 
5. Foreign Address Proof (Driving License/ Electricity Bill/ Telephone Bill)
6. NRO account Cancelled Cheque (with pre-printed name and account number) (Note: for Mutual Funds NRE or NRO account is fine. For Demat only NRO is accepted)
7. Passport Size Photograph

2.  Place of Birth
3.  Annual Income (Rs.): less than 1L/ 1L to 5L/ 5L to 10L/ 10L to 25L/ 25L to 1Cr/ Above 1Cr (To mention applicable slab)
4.  Occupation : Business/ Professional/ Private Sector / Public Sector/ Central Govt./ Housewife/ NGO / Retired/ State Govt./ Statutory Body/ Student/ Others (To mention applicable) 
5. Education: Under-Graduate/ Graduate/ Post-Graduate/ Professional (To mention applicable)
6. Marital Status: Married/Unmarried/Other (To mention applicable) 
7. Mobile Number 
8.  Email-ID
9.  Country of Tax Residency
10.  Tax Identification Number (TIN) or equivalent (pertaining to the country of tax residency). 

Signature Required
